The Professional Regulation Commission (PRC) issued an alert to the public on Sunday, May 24, 2026, regarding a missing unused pre-printed identification card.

Details of the Lost Card

In an official notice, the PRC stated that one unused pre-printed ID card for various professions, bearing Serial Number 25-9545732, has gone missing despite thorough efforts to locate it.

“We hereby inform the public of the loss of one unused pre-printed ID Card for various professions despite diligent efforts to locate and recover the same,” the PRC said.

Warning Against Unauthorized Use

The commission emphasized that the notice aims to prevent any fraudulent, unauthorized, or unlawful use of the accountable form. “Any transaction involving the abovementioned card shall not be honored and shall be considered unauthorized and illegal,” the PRC added.

Call for Public Assistance

The agency urged the public to help recover the missing card by reporting any relevant information to its Procurement and Supply Division (PSD) via email at procurement1@prc.gov.ph or to any PRC office nationwide.

“The public is hereby advised to report any information regarding the said accountable form,” the PRC stated.

The PRC ID is issued to licensed professionals in the Philippines as proof of their legal authority to practice a regulated profession. The loss of a pre-printed ID raises concerns about potential misuse, prompting the commission to act proactively.
